What to Expect with the HOPE Assessment Submission and Reporting Launch in iQIES

On October 1, 2025, the Centers for Medicare & Medicaid Services (CMS) will launch the Internet Quality Improvement and Evaluation System (iQIES) functionality for Hospice Outcomes and Patient Evaluation (HOPE) assessment submission and reports. Below is important information about this transition to iQIES.  

Provider Security Officials (PSOs) Need to Register for an iQIES Account 

Please note that failure to obtain access to iQIES prior to October 1, 2025, will impact your ability to submit HOPE records once the launch is complete. At this time, we are registering PSOs. Additional information will be provided when general onboarding begins.

Outlined below are a few highlights and expectations for the release of the iQIES HOPE assessment submission and reporting functionality. 

Key Highlights of iQIES 

  • Users will be able to securely access iQIES at any time, from any location (provided there is an internet connection).
  • Users will log in once to iQIES. No longer will users be required to log into CMSNet and then into separate applications to upload HOPE records or access reports.
  • Users will have access to tips and information to guide them throughout the HOPE submission process and accessing reports.
  • Users will be allowed to upload HOPE assessments in a similar manner to what was done in the Quality Improvement and Evaluation System (QIES).
  • There will be no User Tool such as the Hospice Abstraction Reporting Tool (HART) for HOPE.  

What to Expect for Providers and Vendors  

  • Hospice Item Set (HIS) records with Target Dates prior to October 1, 2025, can be submitted within QIES through February 15, 2026.  After February 15, 2026, QIES will no longer accept any new or corrected HIS records.
  • HOPE record submissions are only allowed in iQIES.   
  • As of October 1, 2025, both HIS and HOPE assessment deletion requests can be submitted directly within the iQIES System.  HIS assessment deletion requests will be processed in QIES once they have been approved.
  • HIS and HOPE assessment move requests will continue to be processed manually by completing the form(s) found on the iQIES Help webpage, on the Patient Assessment Move/Delete Forms tab.  

QIES / iQIES Report Information

QIES will continue to include detailed information about HIS submissions through 02/15/2026 on the reports listed in the table below.  Beginning 02/16/2025, these reports will become permanently unavailable in the Certification and Survey Provider Enhanced Reporting (CASPER) application. All other reports in the Hospice Provider Reports category in CASPER will be unavailable beginning 10/01/2025 and users must access these reports in iQIES instead.

Following the 10/01/2025 migration into iQIES, users will continue to access some reports or files in CASPER, including the following:  

  • Hospice Review and Correct Report
  • Hospice-Level Quality Measure Report
  • Hospice Patient-Level Quality Measure Report
  • Hospice Provider Preview Reports (November 2025 and February 2026 reports)
  • Hospice CAHPs Provider Preview Reports  (November 2025 and February 2026 reports)
  • FY2026 Non-compliance Notification Letter  

*NOTE: CMS plans a phased migration of the Quality Measure (QM) reports from CASPER into iQIES beginning in February 2026. CMS will send additional information to users in early 2026 with those details. 

Hospice Provider Report Information – iQIES 

Beginning October 1, 2025, users will be allowed to access and run the user-requested Hospice Provider reports in iQIES. As stated above, the only Provider reports that hospices will be allowed to access in QIES are the final validation reports.  Below is some important information about the Provider reports in iQIES: 

  • iQIES will receive HIS-associated record, submission, and error data from QIES on an hourly basis between 10/01/2025 and 02/15/2026. Once the data has been received into iQIES, these data will be available for display in the iQIES Hospice Provider reports.
  • Users’ access to the iQIES reports will be like the access in the CASPER Reporting application, provided that your new iQIES role allows you access to generate and view reports.
  • Users will only be allowed to run reports for the providers to which they have access.
  • In iQIES, users can initially view the report information on the screen and if desired can then download the report to a Portable Document Format (PDF) or Comma-Separated Values (CSV) file.
  • Submission IDs assigned to the submission files by iQIES will have a different sequencing number.  Here are some tips when reviewing reports where QIES and IQIES submission IDs display in the same report:
    • HIS submission files submitted in QIES will have a submission ID of less than 10,000,000.
    • HOPE submission files submitted in iQIES will have a submission ID of more than 10,000,000.
  • Users can schedule reports to run at their desired interval and frequency.

General iQIES Reporting Information 

Patient IDs on Hospice Provider Reports in iQIES 

As part of the HOPE submission and reporting transition, HIS records that had previously been processed and accepted into the QIES Assessment Submission and Processing (ASAP) system with Target Dates of 2015 forward will be migrated into iQIES. As part of this migration, a new unique iQIES state-level patient identifier has been created for each hospice patient, and it will replace the previous QIES ASAP system-assigned Patient ID on all HIS records.  The new state-level patient identifier will also be associated with the HOPE records that are processed in iQIES. 

This new state-level patient ID will be displayed on any Hospice Provider report(s) in iQIES that currently display the Patient ID value.  The new state-level patient ID will also be displayed on the Hospice Quality Measure reports when those are migrated into iQIES. 

  • For example, a patient whose Patient ID was 58608036 as assigned from QIES, will be 298899278 as assigned by iQIES for all reporting and processing purposes.
