Sample IQIES MDS 3.0 Reports

With the migration of Minimum Data Set (MDS) assessment submissions and reporting into iQIES, the Provider and Quality Measure reports that are currently available in the Certification and Survey Provider Enhanced Reporting (CASPER) system will be accessed in iQIES instead.  The migration will begin on Thursday, April 13, 2023 at 8:00 p.m. ET and the new MDS system in iQIES will be available on Monday, April 17, 2023 at 8:00 a.m. ET. 

CMS is providing sample MDS 3.0 Provider reports that will be available following the migration of MDS into iQIES.  The intention of providing these sample reports is to allow software vendors the opportunity to view the layout / contents of the provider-level reports in advance of the migration.  All sample reports contain fictitious or redacted data. 

The sample reports provided are those which are reflective of MDS data submissions. This is not the complete list of reports that will be available following the migration.  Other reports such as the MDS 3.0 Submitter Final Validation, MDS 3.0 Quality Measure, and SNF QRP Quality Measure reports will also be available, but samples of these reports are not being provided.

  • MDS 3.0 Activity
  • MDS 3.0 Admissions/Reentry - Discharges Report
    • This report is a combination of the individual MDS 3.0 Admission/Reentry and MDS 3.0 Discharges reports from CASPER.
    • Users may run the report for Admission events only, Discharge events only, or both types together.
    • The sample report files reflect each version of the report (Admissions only, Discharges only, and Combined).

  • MDS 3.0 NH Error Detail
  • MDS 3.0 NH Final Validation
  • MDS 3.0 Roster
  • MDS 3.0 SB Error Detail
  • MDS 3.0 SB Final Validation

NOTE: the display of the iQIES system-generated NH and SB Validation reports will be the same as the user-requested reports above.

Please note, report data in iQIES will first be displayed in an HTML view.  Users will be allowed to download the report to a PDF or CSV output, if desired.  The zip file labeled “Sample MDS 3.0 iQIES Reports,” listed under the "MDS Vendor Reference & Manuals" section, contains samples of the provider-level reports listed above, both in PDF and CSV output formats.  These samples represent the way that the report data will display in the iQIES reports.
